About Elk Haven Nursing Home and Silver Creek Terrace

Elk Haven is honored to provide nursing services to the residents of Elk County and the surrounding communities for more than 35 years. We believe in compassionate care and strive to improve the quality of life for all those who call our community home. We serve short-stay rehabilitation patients, long-term care residents and personal care residents and believe everyone entrusted to our care deserves to age with grace and dignity. 

Elk Haven Nursing Home is a nonprofit skilled nursing and rehabilitation center. Silver Creek Terrace is a nonprofit personal care community.

We are here to support you and the Elk County community in whatever ways we are able whether you choose Elk Haven and Silver Creek Terrace for you or your loved one's care or not. Our staff is available if you have any questions about the services we offer.

Our facilities have a long-standing commitment to protecting the privacy of our residents. HIPAA is a federal law that offers individuals more control over the use and disclosure of their health information. We are required to make sure that your medical/mental health and financial information that identifies you is kept confidential. We are required to provide you with a Notice of Privacy Practices pursuant to the Health Insurance Portability and Accountability Act of 1996.
